Body Composition & Lean Mass

Body composition outcomes distinguish resistance training from aerobic exercise most clearly: aerobic work tends to reduce total mass, while resistance training preserves or increases lean mass during that process.

A large analysis of aerobic and resistance training in overweight and obese adults compared modality effects on body mass and fat mass, and studies examining combined exercise interventions report greater increases in lean weight and greater reductions in resting blood pressure than exercise alone.

The clinical significance is that lean mass preservation determines resting metabolic rate, functional capacity, and — as the disease and mortality literature shows — prognosis across a range of conditions. Weight alone is a poor proxy for any of these.
